Job summary

Nashua Post Acute is seeking an LPN Weekend Warrior to provide high-quality nursing care on weekends in Nashua, NH. This role offers a base rate of $50 per hour with shift differentials and a schedule of Saturdays and Sundays across 5 weekends on, 1 off in rotation.

Require at least 1 year of LPN experience, active NH LPN license, and valid BLS CPR certification. Daily pay and competitive benefits accompany this weekend position in a fast-paced setting.

Qualifications

Responsibilities

• Provide patient care according to the plan of care and physician orders
• Follow and implement individualized care plans developed by the Nurse Manager and team
• Administer medications and treatments per POC and facility policies
• Monitor residents, report changes promptly to supervising staff
• Maintain accurate and timely documentation per orders and regulatory standards
• Support a safe, respectful, and therapeutic environment for residents and families

$50 Dollars Per Hour Base Rate + Shift Differentials

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of New Hampshire.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
