Job summary

Nashua Post Acute is seeking an LPN Weekend Warrior to provide high-quality nursing care on Saturdays and Sundays with a 5 weekends on, 1 off rotation. The role offers a $50/hour base rate plus shift differentials and daily pay options.

Candidates must have at least 1 year of LPN experience and an active NH license, plus BLS. Shifts run 7am-3pm, 3pm-11pm, and 11pm-7am, with a supportive team and core values of Passion, Respect, and Excellence guiding care.

Qualifications

• Must have at least 1 year of LPN experience.
• Current, active LPN license in New Hampshire.
• Valid BLS certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per standards and plan of care.
• Follow individualized care plans by Nurse Manager and team.
• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes promptly.
• Maintain accurate documentation per physician orders and care plans.
• Support a safe, respectful, therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of New Hampshire.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
