Job summary

WesleyLife Home Health in central Iowa seeks an LPN Visit Nurse to provide in-home nursing care and participate in care planning. You will document visits in the EMR, monitor patient health, and collaborate with a multidisciplinary team to ensure safe, quality care at home.

The role includes on-call rotations, team conferences, and ongoing training, with competitive compensation and benefits. Full-time schedule, Monday–Friday with some on-call coverage.

Qualifications

• Must hold an Iowa LPN license and be eligible to practice.
• Experience in home health or related field preferred.
• Ability to work independently and with a care team.

Responsibilities

• Provide hands-on nursing care guided by individualized care plans.
• Monitor health changes and report updates to the care team.
• Document visits accurately in the EMR for continuity of care.
• Participate in on-call rotations including evenings and weekends.
• Join case conferences, attend meetings and participate in ongoing training.

A Day in the Life as a LPN Visit Nurse with our Home Health team:

• Provide hands‑on nursing care guided by individualized care plans, ensuring each client receives the support they need to remain safe and independent at home.
• Monitor changes in health and well‑being, promptly reporting updates to the care team and ensuring continuity and quality of care.
• Document your visits in the electronic medical record (EMR) with accuracy and clarity, helping the entire team stay connected and informed.
• You’ll participate in on‑call rotations, including evenings, weekends, and holidays—providing urgent support with calm professionalism when it’s needed most.
• You’ll join nursing case conferences, attend team meetings, and complete ongoing training, contributing to a collaborative environment that strengthens care quality and professional growth.

Open Shifts Available:

Full-Time Monday-Friday with rotating on-call (weekends)
