Job summary

WesleyLife Home Health in central Iowa seeks an LPN Visit Nurse to provide in-home nursing care and participate in care planning. You will document visits in the EMR, monitor patient health, and collaborate with a multidisciplinary team to ensure safe, quality care at home.

The role includes on-call rotations, team conferences, and ongoing training, with competitive compensation and benefits. Full-time schedule, Monday–Friday with some on-call coverage.

Qualifications

Must hold an Iowa LPN license and be eligible to practice.
Experience in home health or related field preferred.
Ability to work independently and with a care team.

Responsibilities

Provide hands-on nursing care guided by individualized care plans.
Monitor health changes and report updates to the care team.
Document visits accurately in the EMR for continuity of care.
Participate in on-call rotations including evenings and weekends.
Join case conferences, attend meetings and participate in ongoing training.
