Job summary

WesleyLife Home Health in central Iowa seeks an LPN Visit Nurse to deliver hands-on nursing care in patients' homes, guided by individualized care plans, to promote safety and independence.

The role includes on-call rotations, EMR documentation, and collaborating with a multidisciplinary team. Open shifts are full-time Monday–Friday with rotating on-call (weekends); travel more than 50% of daytime hours is required.

Responsibilities

• Provide hands-on nursing care guided by individualized care plans, ensuring each client receives the support they need to remain safe and independent at home.
• Monitor changes in health and well-being, promptly reporting updates to the care team and ensuring continuity and quality of care.
• Document your visits in the electronic medical record (EMR) with accuracy and clarity, helping the entire team stay connected and informed.
• You’ll participate in on-call rotations, including evenings, weekends, and holidays—providing urgent support with calm professionalism when it’s needed most.
• You’ll join nursing case conferences, attend team meetings, and complete ongoing training, contributing to a collaborative environment that strengthens care quality and professional growth.

A Day in the Life as a LPN Visit Nurse with our Home Health team:
