WesleyLife Home Health is seeking an experienced RN Visit Nurse to provide in-home nursing care across central Iowa counties, helping older adults maintain independence in their own homes. You will perform assessments, support treatment plans, and work with an interdisciplinary team to deliver compassionate, person-centered care.

Qualifications

• Active RN licensure required.
• Experience in home health, wound care, or infusion therapy.
• Ability to form meaningful connections and deliver person-centered care.
• Must pass pre-hire health assessment and 4-panel drug screen.

Responsibilities

As a Visit Nurse (RN), you will play a key role in supporting the health, safety, and independence of the clients we serve. Your responsibilities will include:

Clinical Visits & Assessments

• Perform comprehensive nursing assessments, including recertifications, resumptions of care, discharge assessments, and routine in‑home visits.
• Evaluate client progress, response to treatment, and ongoing needs to ensure high‑quality, person‑centered care.
• Counsel clients and their families on health conditions, care plans, medication management, and other related needs.
• Provide clear, compassionate guidance that empowers families and supports safety and independence at home.
• Maintain ongoing communication with the Nurse Case Manager regarding updates to healthcare provider orders, changes in client condition, incident reports, etc.
• Collaborate with the interdisciplinary team to ensure seamless, coordinated care.
