Job summary

Park Creek Place of North Wales is seeking a Resident Care Coordinator (RCC) with an LPN license to lead the Assisted Living care team and coordinate day-to-day resident care. The role emphasizes scheduling, care plan participation with families, and adherence to company policies to deliver high-quality care.

The RCC will manage staff scheduling, engage with residents and families, and support leadership in operational efficiency, while leveraging basic computer skills and Excel for scheduling

Qualifications

• HS or GED required.
• LPN license required.
• 3–5+ years caregiver experience preferred.
• CNA experience preferred; scheduling and communicating with families.
• Basic computer skills; Excel for scheduling.

Responsibilities

• Assist in managing Assisted Living team members.
• Schedule staff and coordinate shifts.
• Participate in care plan meetings with families and residents.
• Promote company policies and ensure quality care.
• Attend at-risk meetings with leadership team.
• Review and implement operational processes (schedules, caregiver sheets).
