Job summary

Stage Coach LLC (d/b/a Stonebridge Retirement Community) is seeking an experienced LPN to serve as a Resident Care Coordinator (RCC) in a hands-on clinical leadership role. You will balance resident care coordination, medication oversight, and floor support as needed.

This working nurse position emphasizes on-site leadership, direct patient care, and collaboration with families and staff to ensure high-quality care.

Qualifications

• Current Colorado LPN license in good standing (required).
• Experience in Assisted Living, Memory Care, or Long-Term Care preferred.
• Strong medication management and clinical judgment skills.
• Ability and willingness to assist with hands-on resident care.
• Experience supervising or leading care staff strongly preferred.
• Strong documentation and communication skills.
• Ability to remain calm, organized, and decisive in a fast-paced environment.

Responsibilities

• Administer medications and treatments per physician orders and state regulations.
• Serve as RCC by coordinating resident care plans, assessments, and clinical documentation.

This role is hands-on and requires someone who is comfortable balancing resident care coordination, medication oversight, and floor support when needed. This is a working nurse role, not a desk-only position.
