Job summary

Campbell Street seeks a licensed Practical Nurse (LPN) to provide direct, routine care to residents under RN supervision at our Norwalk, IA facility. You will participate in Plan of Care development, document health data, and ensure timely communication with the care team.

Responsibilities include administering medications as ordered, following BON standards, and maintaining patient dignity and privacy. A CPR certification and state LPN license are required for this role.

Qualifications

• LPN license required as per state BON.
• CPR certification required; experience preferred.
• Nursing program approved by state Board of Nursing.

Responsibilities

• Provide residents with the highest quality direct routine care under RN supervision.
• Assist in development and modification of resident Plan of Care and execute delegated interventions.
• Communicate resident needs to primary caregivers and RN; document changes and outcomes.
• Ensure care aligns with BON standards, HIPAA, and Residents’ Bill of Rights.
• Maintain accurate records, monitor patient status, and report changes promptly.
