Job summary

Campbell Street in Granger, IA seeks a motivated Licensed Practical Nurse (LPN) to provide direct routine care under the supervision of the RN. You will participate in care planning and ensure documentation aligns with BON standards and the Residents’ Bill of Rights.

Responsibilities include conducting focused assessments, communicating resident needs to the care team, and maintaining a safe, respectful environment with attention to privacy and dignity.

Qualifications

• Completion of a professional nursing education program approved by the state Board of Nursing.
• Current LPN license in the state where the facility is located.
• Current CPR certification and emergency care training.
• Preferred: experience in skilled nursing and/or assisted living.

Responsibilities

• Provide direct routine care under RN supervision.
• Participate in development and modification of the Plan of Care.
• Ensure compliance with BON standards and HIPAA regulations.
• Communicate resident needs to primary caregivers and document changes.
