Bethesda Health Group is seeking a Licensed Practical Nurse to provide basic nursing care for residents in a long-term care setting in Illinois. The role includes assessment, care planning, and timely documentation, with emphasis on safety and effective communication within the IDT.

The position requires current LPN licensure, IV and CPR/AED certifications, and the ability to manage care for geriatric residents.

Qualifikationen

• LPN licensure in-state or eligible for licensure
• IV certification required or obtainable within six months
• CPR/AED certification required or obtainable within six months
• Ability to assess basic nursing needs of geriatric residents
• Excellent communication and organizational skills
• Strong critical thinking and priority-setting abilities

Aufgaben

• Provide basic nursing care and support assessment on admission and ongoing basis
• Communicate changes in resident condition to the IDT
• Document assessments accurately and timely
• Participate in care planning and update care plans as interventions change
• Administer medications and treatments safely as ordered
• Assist with ADLs and respond to call lights promptly
• Maintain composure during crises and contribute to QAPI-driven problem resolution
• Complete continuing education requirements and stay current with policy updates
