Job summary

Bethesda Health Group is seeking an LPN to provide basic nursing care, assess residents, and coordinate care with the IDT. The role involves documenting, communicating changes, and ensuring safe administration of meds and treatments.

A proactive, compassionate caregiver will support care plans and family education. Active LPN license, CPR/AED and IV certification required or obtainable within six months; LTC experience preferred.

Qualifications

• Education from an accredited school of nursing resulting in LPN licensure
• Current LPN license in state of employment
• CPR and AED certification required or willing to obtain within six months
• IV certification required or willing to obtain within six months
• Long-term care experience preferred
• Ability to assess basic nursing care needs of geriatric residents
• Excellent communication skills with residents, families and staff

Responsibilities

• Provides basic nursing care by assisting in assessment of residents on admission and ongoing basis
• Documents assessments accurately and timely
• Participates in care planning and updates the plan as interventions change
• Administers medications and treatments safely and timely
• Notifies physician using SBARR when changes occur
• Maintains patient safety during crises and monitors care plan implementation
• Completes required nursing continuing education and adheres to Bethesda policies
