Job summary
Community Care, Inc. is hiring a compassionate LPN to provide 1:1 private duty nursing for a 3-year-old boy in Portage, PA. This PRN overnight role offers meaningful, family-centered care from 7:00 PM to 7:00 AM.
You will assist with ADLs, g-tube care, and monitor vitals while delivering safe, compliant care in a home setting. A PA license and CPR certification are required, with 6 months of nursing experience preferred.
Qualifications
- Current LPN or RN license in Pennsylvania.
- Graduate of an accredited nursing program.
- CPR certification.
Responsibilities
- Assist with Activities of Daily Living (ADLs).
- Provide trach/vent care and monitoring.
- Monitor vital signs and respiratory status.
- Administer medications safely and accurately.
- Perform g-tube care/feedings as needed.
- Communicate clearly with the family and care team.
- Deliver 1:1 private duty pediatric care.

- Assist with Activities of Daily Living (ADLs)
- Perform g-tube care/feedings
- Monitor vitals
- Trach/vent care/management
- Perform respiratory assessments
- Administer medications safely and accurately
- Communicate clearly and compassionately with the care team and family
- Deliver exceptional, family‑centered pediatric care
- Current LPN (or RN) license in the state of Pennsylvania
- Graduate of an accredited nursing program
- At least 6 months of nursing experience (home health preferred, but not required)
- CPR certification
- Reliable transportation and a valid driver's license
- A caring heart, strong independence, and a passion for pediatric care
