Job summary
Community Care, Inc. in Portage, PA is seeking a compassionate LPN or RN to provide one-on-one private duty nursing for a 3-year-old male child. This PRN overnight role runs 7:00 PM–7:00 AM, offering meaningful, family-centered pediatric care in the home setting.

Qualifications
- Current LPN or RN license in Pennsylvania.
- Graduate of an accredited nursing program.
- At least 6 months of nursing experience (home health preferred).
- CPR certification.
- Reliable transportation and a valid driver’s license.
- Passion for pediatric care and a caring heart.
Responsibilities
- Assist with activities of daily living (ADLs).
- Provide g-tube care and feedings.
- Monitor vitals on shift.
- Trach/vent care and management.
- Perform respiratory assessments.
- Administer medications safely and accurately.
- Communicate clearly with the care team and family.
- Deliver family-centered pediatric care.

Community Care, Inc. is looking for a compassionate and dedicated LPN to provide 1:1 private duty nursing care for a wonderful 3-year-old male child. - Assist with Activities of Daily Living (ADLs)
- Perform g-tube care/feedings

- Communicate clear and compassionately with the care team and family
- Deliver exceptional, family-centered pediatric care
- Current LPN (or RN) license in the state of Pennsylvania
- Graduate of an accredited nursing program
- At least 6 months of nursing experience (home health preferred, but not required)
- CPR certification
- Reliable transportation and a valid driver's license
- A caring heart, strong independence, and a passion for pediatric care
