Responsibilities

• Provide direct clinical care and assist with diagnostic testing and vaccinations.
• Manage administrative tasks such as hospital discharge follow-ups and charting in EHR.
• Support Care Team Huddles and maintain accurate documentation per protocols.
• Assist other MOA responsibilities to ensure seamless patient care.
• Adhere to DOH regulations and clinic policies.
