Job summary

New York Health (NYH) in Brooklyn, NY, is seeking a Licensed Practical Nurse (LPN) to join our multidisciplinary care team. The role emphasizes accurate EMR documentation, medication reconciliation, patient education, and collaboration with physicians and staff to support optimal patient outcomes.

You will manage inbox messages, assist with exams, prepare injections, perform phlebotomy and point-of-care testing, and ensure clean, ready exam rooms.

Qualifications

• Current NYS LPN license is required.
• Basic Life Support (BLS) certification is required.
• IV Competency required.
• Strong attention to detail and professional conduct.
• Excellent verbal and written communication with patients and staff.
• Experience with EMR systems.

Responsibilities

• Update and maintain the electronic medical record with up-to-date vital signs, patient demographics, medication reconciliation, allergies and medical diagnoses.
• Manage Physician inbox messages and prioritize tasks assigned via the electronic medical record.
• Assist in maintaining the medical inventory; place orders for new materials and stock rooms to ensure adequate supplies for physician staff.
• Clean and prepare examination rooms prior to patient appointments.
• Escort patients to exam rooms.
• Provide patient education concerning medications, management of diseases, home treatments, and special diets.
• Assist physician with exams and/or procedures.
• Prepare and administer injections.
• Phlebotomy and collection of other lab specimens.
• Perform point of care lab testing.
• Perform EKGs.
• Administer and document medication under a physician’s order and follows the five (5) medication rights and reduce the potential for medical error.
• Be cross-trained in infusion & intake.
