Joseph P. Addabbo Family Health Center in New York, NY seeks a Licensed Practical Nurse to support non-professional and paraprofessional health services under supervision of the Nursing Director.

You will provide direct patient care under supervision and help implement patient care plans within a collaborative team. Responsibilities include documenting data for provider assessment, maintaining logs, following infection control standards, and assisting with vaccine administration.

Formación

• Certificate or diploma from an accredited LPN program.
• 1–3 years nursing experience; ambulatory care preferred.
• Excellent communication and teamwork skills.
• Experience with EMR and accurate data entry.

Responsabilidades

• Provide direct patient care under supervision of the Nursing Director.
• Document data to assist provider assessment and planning; track referrals.
• Deliver care per patient care plan and organizational policies.
• Maintain infection control standards to protect patients and staff.
• Assist with vaccine storage, administration and safety practices.
• Participate in quality improvement and committee activities.
• Keep nursing areas, rooms and equipment orderly and sterile.
• Prepare supplies and manage inventory for clinic operations.
• Support administrative staff by registering patients and recording data.
• Attend conferences and agency meetings to stay updated on policies.
