Job summary

Community Healthcare Network is seeking a Full-Time LPN I in New York, NY to support multiple centers and health providers in delivering clinical care and patient education.

The LPN will administer medications, perform venipuncture, and assist with procedures while ensuring patient safety and privacy. A NYS LPN license and CPR certification are required, with CPR training within six months if not current.

Qualifications

• Must hold a New York State License to practice as an LPN.
• CPR certification required within six months of employment.
• High School Diploma or GED required.

Responsibilities

• Coordinates with healthcare team to provide high quality patient care.
• Administers medications and injections accurately and documents responses.
• Performs venipuncture, intakes patients, and assesses health needs.
• Orders medications, vaccines, and supplies as needed.
• Maintains proper refrigeration and emergency supply checks.
• Assists providers with procedures and interprets regimens when needed.
• Provides health counseling, including HIV counseling where appropriate.
• Maintains confidentiality and privacy of patient information.
• Works shifts across multiple centers as required by CHN policy.
