Job summary

Services for the UnderServed is seeking a Licensed Practical Nurse to join an Assertive Community Treatment team in New York. The LPN will supervise and support the RN, administer injections, monitor vital signs, and coordinate medical care with community providers.

You will maintain progress notes, enter data in the EHR, and advocate for clients to access integrated physical and mental health services while upholding confidentiality and safety standards.

Qualifications

• Bachelor or Associate in Nursing and NY license.
• Experience in mental health services preferred.
• Ability to coordinate care with RNs and psychiatrists.

Responsibilities

• Administer IM injections and provide medications as prescribed.
• Meet clients in the community to monitor health status.
• Maintain progress notes and document in EHR promptly.
• Coordinate with emergency services when needed.
• Support RN and psychiatrist in medical care coordination.
