Job summary

Brooklyn Plaza Medical Center in Brooklyn, NY is seeking a Licensed Practical Nurse to perform triage, assessments, education, charting, and injections. The LPN will support the HIV Clinical Coordinator and collaborate with the care team to coordinate patient care.

Requirements include an active NY LPN license, ADN or equivalent, and at least two years in ambulatory care, with HIV/AIDS knowledge strongly preferred. Weekend and late shift availability may be required.

Qualifications

• Possession of a current and valid New York State License as a Practical Nurse, and evidence of education equivalent to the ADN.
• Minimum two years of direct LPN experience in ambulatory care, preferably in primary care.
• Experience and knowledge of HIV/AIDS essential.

Responsibilities

• Assist in triage, physical assessment, patient education, and charting.
• Administer prescribed medications and injections.
• Counsel patients individually and in groups on health education.
• Maintain safe, comfortable, and dignified patient care environment.
• Supervise ancillary staff under the RN/Director of Nursing.
• Document care in the patient’s chart (EHR).
• Monitor vital signs and perform procedures such as venipuncture and EKG.
