Job summary

Crossover Health is seeking a full-time Licensed Practical Nurse (LPN) for our New York clinic. The role focuses on delivering patient-centered care, coordinating with a multidisciplinary team and supporting member experiences in a fast-paced environment.

Responsibilities include patient check-ins, vitals, phlebotomy, immunizations, and guiding patients through the care process within the self-insured benefits model.

Qualifications

• Licensed Vocational Nurse with a minimum of 3 years of clinical experience.
• Current BLS certification.
• Ability to coordinate patient care and navigate a fast-paced clinic.

Responsibilities

• Delivers patient care under the oversight of an RN or clinic supervisor.
• Collaborates with the care team to facilitate a patient-centered medical home.
• Take medical histories, record vitals, and assist clinicians.
• Coordinates and connects patients with specialists and facilities.
• Provides phlebotomy and immunization services.
• Assists with member check-in and clinic workflows.
