Job summary

Stillwater Hospice in Fort Wayne, IN, is seeking a Phone Nurse to support the coordination of patient care. You will receive and make patient-related calls, relay changes in care plans to physicians and the interdisciplinary team, and assist with admissions as needed.

Qualified candidates will have an Indiana nursing license, a valid driver’s license, and hospice or palliative care experience. Strong communication and documentation skills are required to ensure high-quality patient care.

Qualifications

• Graduate of an accredited school of nursing; RN or LPN license in Indiana.
• Must have a current license to practice in Indiana.
• Must have a valid driver’s license and access to a car with insurance.
• Hospice/palliative care experience preferred.
• Excellent communication and teamwork skills are essential.

Responsibilities

• Receives and makes patient-related telephone calls to coordinate care.
• Communicates patient needs to physician, family, and interdisciplinary team.
• Documents findings accurately in the electronic documentation system.
• Coordinates care with providers, pharmacies, and equipment vendors.
• Supports admissions and intake as needed.
• Delivers direct nursing care and patient/family teaching when required.
