Job summary

Stillwater Hospice is seeking an On-Call RN to provide nursing care for hospice patients in their homes across Northeast Indiana. The role includes on-call coverage, holiday and weekend shifts, and collaboration with the clinical team leader to ensure optimal patient outcomes.

The ideal candidate is an Indiana-licensed RN, CPR certified, with hospice or home health experience preferred, and a strong interpersonal approach to support patients and families during palliative care.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing preferred.
• Must have a current RN license to practice in Indiana.
• Must hold current CPR Certification or obtain it within the orientation period.
• Prefer one (1) year of recent hospital or home health care experience.
• Must be able to provide those services requiring substantial specialized nursing skills.
• Must be able to safely perform the job's essential functions without posing a direct threat to the health and/or safety of him/her or others in the workplace.
• Must be comfortable working with hospice patients, emphasizing palliative care.
• Strong interpersonal skills.
• Must have an Indiana motor vehicle operator’s license and have access to an automobile.

Responsibilities

• Provision of nursing care while on call for patients by assessing needs, making appropriate revisions, and evaluating care outcomes.
• Deliver nursing care to patients, including teaching, direct care, case management, referral, advocacy, pain and symptom management, and initiating appropriate interventions.
• Supervises clinically the care delegated to LPNs, Home Health Aides, and CNAs and documents same as needed.
• Assists with in-servicing the community on the hospice philosophy as needed/requested.
• Coordinates care and services with other health care providers.
• Maintains confidentiality of personal matters and the Agency's business.
• Participates in quality improvement activities and/or other agency committees when requested.
