Job summary

Gurwin Jewish ~ Fay J. Lindner Residences in Commack, NY is seeking an LPN to join our luxurious assisted living team on a per diem basis, working Friday and Saturday nights from 11:00 pm to 7:15 am.

You will deliver direct patient care, administer medications, monitor vital signs, and support residents with compassionate, person-centered care. This role emphasizes teamwork with an interdisciplinary healthcare team, accurate charting, wound care, and family education, in a refined setting that

Qualifications

• Current NY LPN license or licensure.
• Prior experience in long-term care or assisted living preferred.
• Strong clinical skills in geriatric care and wound care.

Responsibilities

• Provide direct patient care including medications, vital signs, and wound care.
• Develop and implement individualized care plans with the healthcare team.
• Maintain accurate and detailed patient records.
• Participate in interdisciplinary team meetings for comprehensive care.

About the Role:

We are seeking a dedicated and compassionate LPN to join our team on a per diem basis, working every Friday and Saturday from 11pm-7:15am. As an LPN at Gurwin Jewish ~ Fay J. Lindner Residences, you will have the opportunity to provide exceptional care to our residents while enjoying a unique and luxurious assisted living community.
