Job summary

Gurwin Jewish ~ Fay J. Lindner Residences in Commack, NY seeks an LPN Shift Supervisor to lead care for residents in a luxurious Long Island assisted living community. The role requires current NY LPN licensure and strong clinical oversight.

Shifts Tuesday to Saturday 3:00 p.m. - 11:15 p.m. with a $10,000 sign-on bonus; you will supervise LPNs and HHAs, conduct rounds, maintain records, and participate in quality improvement efforts.

Qualifications

• Current LPN licensure in the State of New York.
• Minimum of two years of experience in long-term care or assisted living.
• Proficiency in electronic medical records (EMRs).

Responsibilities

• Supervise and coordinate the work of LPNs and HHAs to ensure efficient and effective delivery of care.
• Conduct daily rounds to ensure resident care is being provided in accordance with established protocols.
• Maintain accurate records and reports of resident care and service.
• Participate in Quality Improvement initiatives to ensure continuous improvement of care and services.
• Collaborate with other departments to ensure seamless transitions and communication.

As an LPN Shift Supervisor at Gurwin Jewish ~ Fay J. Lindner Residences, you will be responsible for ensuring the highest level of care and service to our residents. Your duties will include:

• Conducting daily rounds to ensure resident care is being provided in accordance with established protocols
• Identifying and addressing resident needs, concerns, and complaints
• Maintaining accurate records and reports of resident care and service
• Participating in Quality Improvement initiatives to ensure continuous improvement of care and services
• Collaborating with other departments to ensure seamless transitions and communication
