Job summary

Gurwin Jewish : Fay J. Lindner Residences is seeking a dedicated LPN to join our team on a per diem basis, working every Friday and Saturday from 11pm-7:15am.

You will provide direct patient care, administer medications, and collaborate with the healthcare team to ensure high-quality geriatric care. This role offers a luxurious assisted living community, a supportive work environment, and the opportunity to make a meaningful difference in residents' lives while maintaining strong clinical skills

Qualifications

• Current LPN licensure in New York.
• Experience in long-term care or assisted living preferred.
• Strong geriatric care knowledge and clinical skills.

Responsibilities

• Provide direct patient care including administering medications and vitals.
• Develop and implement individualized care plans with the healthcare team.
• Maintain accurate and detailed patient records.
• Participate in interdisciplinary team meetings for comprehensive care.
• Collaborate with other professionals to ensure seamless transitions and quality care.
• Educate and support residents and families.
• Maintain a safe and clean environment for residents and staff.
