Job summary

Gurwin Healthcare System is seeking a part-time LPN to join our care team at our 36-acre campus in Commack, NY. You will work every other Friday and Saturday, 11:00p.m. - 7:15a.m., delivering high-quality geriatric nursing care with a supportive interdisciplinary team.

Current NY LPN licensure is required; prior long-term care or assisted living experience is preferred. We offer a competitive salary and a comprehensive benefits package in a dynamic work environment.

Qualifications

• Current LPN licensure in the state of New York.
• Prior experience in long-term care or assisted living preferred.
• Geriatric care knowledge is important.
• Strong communication and interpersonal skills.
• Ability to work effectively as part of a team.

Responsibilities

• Supervising and coordinating the work of LPNs and HHAs to ensure efficient and effective delivery of care.
• Provide direct patient care to our residents, including administering medications, performing vital signs, and providing wound care.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Maintain accurate and detailed patient records.
• Participate in interdisciplinary team meetings to ensure comprehensive care.
• Collaborate with other healthcare professionals to ensure seamless transitions and quality care.
• Provide education and support to residents and their families.
• Maintain a safe and clean environment for residents and staff.

Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J.
