Gurwin Healthcare System in Commack, NY, is seeking a per diem Licensed Practical Nurse to join our compassionate geriatrics team. You will provide skilled nursing care and coordinate with nursing leadership to ensure resident safety and comfort.

The role emphasizes supervision of Medication Aides and Resident Care Coordinators, timely shifts reporting, and clear communication with families and physicians. Per diem shifts offer flexible scheduling within our 36‑acre campus.

Formación

• Current LPN licensure in the state of New York.
• Prior experience in long-term care or assisted living preferred.
• Strong clinical skills and knowledge of geriatric care.

Responsabilidades

• Supervision of trained Medication Aides.
• Supervision and assignment of Resident Care Coordinators.
• Maintenance of the 24 hour shift report.
• Inform family and MD of health/behavior changes when Director of Resident Services is absent.
• In emergencies, pass medications to residents and verify narc counts.
• Other related duties as directed.

About this role:

You will pick up shifts as needed.

Requirements:

• Current LPN licensure in the state of New York
• Prior experience in long-term care or assisted living preferred
• Strong clinical skills and knowledge of geriatric care
• Excellent communication and interpersonal skills
• Ability to work effectively as part of a team

Responsibilities include, but are not limited to:

• Supervision of trained Medication Aides
• Supervision and assignment of Resident Care Coordinators
• Supervision and maintenance of the 24 hour shift report
• Apprise family and M.D. of changes in resident’s health and behavior in the absence of the Director of Resident Services
• In an emergency, will pass medications to the residents
• Perform and verify NARC Count – Beginning and end of each shift
• Other related duties as directed
