Job summary

AMEDISYS HOME HEALTH is seeking an Intake/Scheduling Coordinator to manage home-visit scheduling for field staff and coordinate patient care under the supervision of the Clinical Manager.

The role requires an active nursing license in the assigned state, a high school diploma or GED, and at least one year of administrative experience in healthcare. Strong scheduling skills and attention to capacity are essential.

Qualifications

• Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to that state the employee is assigned to work by the company.
• High School diploma or GED equivalent.
• One (1+) year of administrative experience in a health care environment.

Responsibilities

• Schedules patient visits and coordinates with field staff and Clinical Manager.
• Prepares clinician schedules and monitors capacity to meet productivity standards.
• Assigns clinicians efficiently by geography and required care, optimizing capacity.
• Ensures missed visits are rescheduled or escalated to the Clinical Manager.
• Handles scheduling-related calls from patients and field staff; refers issues as needed.
• Monitors hospitalized patients and ensures proper follow-up by the team.
