Qualifikationen

• Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensed in the state of practice.
• At least 1 year of home health experience.
• Coding certification is preferred.
• Valid state driver’s license and auto liability insurance required.
• Excellent communication and strong organizational skills; ability to manage multiple tasks.

Aufgaben

• Schedule visits for patients and coordinate with field staff, physicians, and patients.
• Initiate infection control forms and handle required reporting.
• Back up during lunch hours and busy times; assist with weekly case conferences.
• Maintain hospitalization log and coordinate notes across office and field staff.
• Prepare and adjust weekly schedules; ensure visits are arranged per case requirements.
• Run scheduling reports and review productivity and geographic issues.
• Review visit notes in scheduling system and assist with patient transfers between branches.
• May perform patient visits or participate in on-call rotations as needed.
