Job summary

Humana Inc presents CenterWell Home Health’s Patient Services Coordinator-LVN role. You will coordinate scheduling and communications among patients, field staff, and physicians to ensure seamless care.

The position also supports daily office operations and staff management. Requirements include an active LVN/LPN license in the state of practice and at least 1 year of home health experience, with strong communication and organizational skills.

Qualifications

• Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
• At least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.
• Valid state driver's license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Excellent communication, organization, and multitasking abilities.

Responsibilities

• Schedules visits for patients and coordinates with field staff, patients, and physicians to maintain care continuity.
• Assists with day-to-day office and staff management.
• Back-up support during lunch hours and busy times, answering field calls and assisting with weekly case conferences.
• Maintains hospitalization log and coordinates notes with office staff and sales teams.
• Manages scheduling adjustments and rescheduled visits as needed.
• Verifies visit paper notes and assists with internal patient transfers between branches.
