Job summary

CenterWell Home Health is seeking an experienced Patient Services Coordinator-LPN in West Virginia. You will manage patient schedules, coordinate visits, and ensure timely reassignment when staff are unavailable, supporting field staff and case conferences.

This role requires an active RN/LPN license in the state, at least 1 year of home health experience, a valid driver’s license, and strong communication and organizational skills.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which he / she practices.
• Have at least 1 year of home health experience.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.

Responsibilities

• Manage schedules for all patients and reassign if needed.
• Initiate infection control forms and coordinate with HRD/upload in worker console.
• Provide backup during lunch and assist with case conferences; refer clinical questions to Branch Director.
• Maintain hospitalization log and coordinate with staff; send electronic log to office, field, and sales staff.
• Schedule discharge visits, OASIS collection, and add-ons; delete remaining schedules.
• Reschedule declined or missed visits; process reassigned/rescheduled visits.
• Ensure supervisory visits are scheduled; run scheduling reports and prepare weekly Agent Schedules.
• Verify visit paper notes in scheduling console; assist with internal transfer of patients between branches.
• If clinical, may perform patient visits or participate in on-call rotation.
