Job summary

CenterWell Home Health is seeking a Patient Services Coordinator LPN in Charlotte to manage patient visit scheduling and coordinate with field staff, patients, and physicians. The role includes back-office tasks, logging visits, and supporting weekly case conferences.

Qualified candidates are LPN/LVN with at least 1 year of home health experience, valid driver's license, and strong communication and organizational skills. Some clinical tasks may be required, with on-call rotation possible.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state of practice.
• At least 1 year of home health experience.
• Must possess a valid state driver's license and automobile liability insurance.
• Must be licensed in the state of employment if applicable.
• Coding certification is preferred.

Responsibilities

• Schedule visits and manage patient and field staff calendars.
• Edit schedules for staff shortages and reassign patients promptly.
• Maintain infection control forms and log files as needed.
• Assist with office operations and backup coverage during peak times.
• Process and review lab reports and coordinate with medical staff when clinical.
