Job summary

CenterWell Home Health is seeking a Nurse Scheduler/Coordinator (LPN/LVN or RN) to manage patient visit schedules, coordinate with field staff, and maintain care continuity in our Charlotte, NC region. The role supports day-to-day office operations and involves collaboration with physicians, case managers, and other staff.

The position requires 1+ year home health experience, state licensure, a valid driver’s license, and strong communication skills.

Qualifications

• Be a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensed in the state.
• Have at least 1 year of home health experience.
• Must possess excellent communication and organizational skills.
• Must have valid state driver’s license and automobile liability insurance.

Responsibilities

• Manage patient schedules and update the worker console.
• Initiate infection control forms and route to HRD.
• Assist field staff and participate in weekly case conferences.
• Maintain the client hospitalization log and coordinate notes.
• Schedule visits to cover skilled nurse/injection visits.
• Prepare weekly schedules and review productivity/geography for approval.
• Process lab reports and route to Medical Records.
• May perform patient visits or participate in on-call rotation.
