Job summary

CenterWell Home Health is seeking a Patient Services Coordinator, LPN to schedule visits and coordinate care with field staff, patients and physicians. The role supports daily office operations, staff management, and ensures timely reassignment of visits.

The ideal candidate is a licensed nurse with at least 1 year of home health experience, excellent communication, and strong organizational skills. This is a full-time role based in Texas with 40 hours per week.

Qualifications

• Must be licensed as a Licensed Professional Nurse (LPN) or Licensed Vocational Nurse (LVN) in the practicing state.
• At least 1 year of home health experience.
• Packet review / QI experience preferred.
• Coding certification is preferred.
• Valid state driver’s license and automobile liability insurance.
• Must be licensed in the state of employment if applicable.
• Excellent communication, organizational, and multitasking skills.

Responsibilities

• Schedule visits and coordinate care with field staff, patients, and physicians.
• Assist with day-to-day office and staff management and scheduling reports.
• Maintain patient logs, coordinate transfer of patients between offices, and support case conferences.
• Handle clinical tasks like reviewing lab reports and routing to the appropriate staff.
• Provide coverage during peak times and assist with on-call rotation when needed.
