Job summary

CenterWell Home Health is seeking a dedicated Patient Services Coordinator-LPN to manage visit scheduling and coordinate with field staff, patients, and physicians to maintain care continuity. This role supports day-to-day office operations and staff management.

The candidate will handle scheduling, clinical coordination, and reporting, with a focus on efficient patient care and smooth branch operations, working in a full-time capacity.

Qualifications

• Must hold a valid state RN or LVN license.
• Minimum 1 year of home health experience.
• QI / packet review experience preferred.
• Coding certification preferred.
• Valid driver's license and auto liability insurance.
• Active licensure in the state of employment.

Responsibilities

• Manage schedules for all patients and edit schedules as needed.
• Coordinate with field staff, physicians, and office for timely visits.
• Assist with on-call rotation and attend weekly case conferences.
• Process lab reports and route to appropriate teams.
• Run scheduling reports and prepare weekly agent schedules.
• Ensure supervisory visits are scheduled and documented.
