Job summary

Community Care, Inc. is seeking a compassionate LPN to provide 1:1 private duty nursing for a 13-year-old male child in Monongahela, PA. This weekend overnight role supports ADLs, trach/vent care, and mobile, family-centered pediatric care.

What You'll Do

• Assist with Activities of Daily Living (ADLs)
• Trach/vent care/monitoring
• Monitor vitals
• Administer medications safely and accurately
• Communicate clearly and compassionately with the care team and family
• Deliver exceptional, family‑centered pediatric care

What You'll Need

• Current LPN (or RN) license in the state of Pennsylvania
• Graduate of an accredited nursing program
• At least 6 months of nursing experience (home health preferred, but not required)
• CPR certification
• Reliable transportation and a valid driver's license
• A caring heart, strong independence, and a passion for pediatric care
