Job summary

Community Care, Inc. is seeking a compassionate LPN to provide 1:1 private duty nursing for a 13-year-old male child in a home setting, focusing on Trach/Vent care.

This overnight weekend role offers Fridays and Saturdays, 6:00 PM to 6:00 AM, with 1:1 support and a collaborative care team.

You will monitor vitals, administer medications safely, and partner with family to deliver family-centered pediatric care.

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate from an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Trach/vent care/monitoring.
• Monitor vitals.
• Administer medications safely and accurately.
• Communicate clearly and compassionately with the care team and family.
