Job summary

Community Care, Inc. in Washington, PA is seeking a compassionate LPN to provide one-on-one pediatric private duty nursing for a 12-year-old male with Trach/Vent needs. This full-time overnight role offers 10:00 PM–6:00 AM shifts and a supportive home-care environment.

You will perform ADLs, g-tube care, vitals monitoring, seizure observation, and safe medication administration while coordinating closely with the family and care team.

Qualifications

• LPN license in Pennsylvania required.
• Minimum 6 months nursing experience, home health preferred.
• CPR certification required.

Responsibilities

• Assist with activities of daily living (ADLs).
• Provide g-tube care and feedings.
• Trach/vent care and monitoring.
• Monitor vitals and respond to changes.

What You’ll Do

What You’ll Need

• Current LPN (or RN) license in the state of Pennsylvania
• Graduate of an accredited nursing program
• At least 6 months of nursing experience (home health preferred, but not required)
• CPR certification
• Reliable transportation and a valid driver's license
• A caring heart, strong independence, and a passion for pediatric care
