Job summary

Community Care, Inc. is seeking a compassionate LPN to provide 1:1 private duty nursing for a 32-year-old female adult in Uniontown, PA. This full-time evening role offers one-on-one care in a home setting.

You will monitor trach/vent, assist with ADLs, administer medications, and collaborate with the care team and family. Evening schedule is Monday through Friday, 3:00 PM–11:00 PM; overnights and weekends may be available.

Qualifications

• Current LPN (or RN) license in Pennsylvania.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification is required.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform trach/vent care and monitoring.
• Administer medications safely and accurately.
• Communicate clearly with the care team and family.
• Deliver family‑centered client care.
