CCH Healthcare is seeking an MDS Coordinator (RN or LPN) for our Cincinnati area facilities. You will oversee the RAI process, ensure timely MDS assessments, CAAs, and care plans, and support accurate Medicare PPS and Medicaid reimbursements.
Ideal candidates hold an active Ohio RN or LPN license, with at least 2 years of MDS experience in a long-term care setting, CPR certification, and a proactive, detail-oriented approach to interdisciplinary collaboration.

CCH Healthcare is currently hiring experienced MDS Coordinators for our Cincinnati area facilities. We’re looking for passionate and detail-oriented nurses who excel in clinical documentation, team collaboration, and resident-centered care.
As an MDS Coordinator, you will be responsible for overseeing the entire RAI process to ensure timely and accurate completion of MDS assessments, Care Area Assessments (CAAs), and care plans. This role is essential to compliance, clinical excellence, and reimbursement accuracy within our facility.
Coordinate and manage MDS assessment and submission processes
Ensure accuracy and timely state transmission of MDS data and reports
Lead interdisciplinary care plan meetings
Monitor Quality Measures (QMs) and provide data-driven insights
Support accurate Medicare PPS and Medicaid reimbursement processes
Active, unencumbered RN or LPN license in the State of Ohio
Minimum 2 years of MDS experience in a long-term care setting (required)
CPR certification
RAC-CT certification preferred
Strong knowledge of Case-Mix, Medicare PPS, and regulatory compliance
Excellent attention to detail and analytical abilities
Strong organizational and communication skills
Ability to lead and collaborate with interdisciplinary teams
