Edmonds Post Acute is seeking a dedicated MDS Assistant (LPN) in Edmonds, WA to support timely MDS assessments and care planning under the RAI process. The role emphasizes documentation accuracy, chart audits, and care coordination within an interdisciplinary team.

You will collaborate with the MDS Coordinator to ensure deadlines are met, participate in care plan meetings, and use our EMR for data entry. Competitive hourly pay reflects experience and qualifications.

Qualifikationen

• Active Washington State LPN license is required.
• At least 1 year of long-term care or geriatric nursing experience.
• Working knowledge of RAI/MDS, PDPM, and payer sources (Medicare, Medicaid, HMO, private pay, hospice).
• Comfort with EMR systems and data entry; clerical experience is a plus.
• Strong attention to detail and ability to manage deadlines.
• Clear communication with residents, families, staff, and external agencies.
• HIPAA knowledge and commitment to resident confidentiality.
• Ability to stand, walk, move throughout the building, and lift up to 25 pounds.
• Annual TB screening and physical exam per facility policy.

Aufgaben

• Assist the MDS Coordinator with timely, accurate completion of MDS assessments under the RAI process.
• Help develop and update resident care plans in line with current RAI guidelines and required timeframes.
• Maintain assessment schedules and track deadlines for MDS submission to the state.
• Conduct ongoing chart audits to confirm documentation supports assessments and care plans.
• Audit records for proper physician and family notification, and complete notifications when assigned.
• Enter assessment data and documentation in our EMR.
• Attend Interdisciplinary Care Plan meetings and talk with residents and families during care plan reviews.
• Work with every department to make sure resident needs are captured and communicated.
