Job summary

Ideal candidates will have an RN or LPN license, CPR certification, and at least 6 months of long-term care experience. The role emphasizes collaboration, data accuracy, and continuous quality improvement.

Qualifications

• Graduate of a LPN or RN program.
• Active State LPN or RN license.
• Six months experience in a long-term care environment preferred.

Responsibilities

• Manage MDS assessments based on CMS guidelines.
• Coordinate, record, update, and monitor all MDS and PPS data.
• Participate in clinical meetings and patient/family care conferences.
• Coordinate timely development of resident assessment (MDS) and care plans.
• Communicate with residents, families, medical staff, and payors.
• Support staff education on documentation and quality initiatives.
• Monitor quality metrics and ensure accuracy of reporting.
