Job summary

Edmonds Post Acute is hiring an MDS Assistant (LPN) to support the MDS Coordinator in keeping resident assessments accurate and on time. You will use clinical knowledge to shape care plans, audit charts, and ensure documentation meets state and federal requirements.

You will work with an interdisciplinary team to collect data, verify records, and participate in care plan reviews. Competitive pay reflects experience, with benefits and growth opportunities available on site.

Qualifications

• Active Washington State LPN license required.
• At least 1 year in long-term care or geriatric nursing.
• Knowledge of MDS/RAI, PDPM, and payer sources (Medicare/Medicaid/HMO).
• Comfort with EMR systems and data entry; clerical experience is a plus.
• Strong attention to detail and deadline management.
• Clear communication with residents, families, staff and outside agencies.
• HIPAA knowledge and resident confidentiality commitment.
• Ability to stand, walk, move throughout building, lift up to 25 pounds.
• Annual TB screening and physical per facility policy.

Responsibilities

• Assist MDS Coordinator with timely, accurate MDS assessments under the RAI process.
• Help develop and update resident care plans per current RAI guidelines.
• Maintain assessment schedules and track MDS submission deadlines.
• Conduct ongoing chart audits to verify documentation supports assessments.
• Audit records for physician and family notification; complete notifications as assigned.
• Enter assessment data in the EMR and maintain records.
• Attend Interdisciplinary Care Plan meetings and communicate with residents and families.
• Collaborate with all departments to capture resident needs accurately.
