Job summary

Cascadia Healthcare is seeking an MDS Coordinator (LPN) in Washington to ensure accurate assessments and care plans meet federal and state guidelines. You will identify resident acuity, coordinate MDS timing, and communicate needs to families and staff.

The role requires an active LPN license, CPR certification, and at least six months in long-term care. You will participate in clinical meetings and support documentation and quality assurance initiatives.

Qualifications

• Knowledge of CMS guidelines for MDS.
• Ability to implement Nursing department programs and procedures.
• CPR certification required and kept current.

Responsibilities

• Manage MDS assessments per CMS/PPS rules.
• Coordinate, update and monitor all MDS datasets.
• Participate in clinical meetings and care conferences.
• Coordinate communications among residents, families, medical and administrative staff.
• Ensure timely development and completion of resident care plans.
• Review quality metrics and support quality improvement efforts.
