Job summary

The Licensed Practical Nurse at Talented Medical Solutions delivers nursing care to residents in a long-term care setting under the direction of a Registered Nurse or physician. Administers medications and treatments, monitors resident status, and supervises the work of Certified Nursing Assistants.

Responsibilities include accurate documentation, communicating changes to the care team, and following infection control and safety guidelines to ensure regulatory compliance in a long-term care

Qualifications

• Graduate of an accredited practical or vocational nursing program (Required).
• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) licensure in the state of practice (Required).
• Basic Life Support (BLS) certification (Required).
• Prior experience in long-term care or skilled nursing settings (Preferred).

• Perform resident assessments and monitor for changes in physical, cognitive or behavioral status.
• Supervise and delegate appropriately to Certified Nursing Assistants on the assigned unit.
• Complete accurate and timely documentation in the resident record and medication administration record.
• Communicate changes in resident condition to the Registered Nurse, physician and family as appropriate.
• Follow infection control, safety and regulatory requirements governing long-term care practice.
