Job summary

Talented Medical Solutions is seeking a Licensed Practical Nurse to provide nursing care for residents in a long-term care setting under the direction of an RN or physician.

The role involves administering medications, conducting resident assessments, supervising certified nursing assistants, and maintaining accurate documentation while communicating changes to the care team and family.

A current LPN/LVN license and BLS certification are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Prior experience in a long-term care or skilled nursing setting is preferred.

Responsibilities

• Administer medications and treatments in accordance with physician orders and facility policy.
• Perform resident assessments and monitor for changes in physical, cognitive or behavioral status.
• Supervise and delegate appropriately to Certified Nursing Assistants on the assigned unit.
• Complete accurate and timely documentation in the resident record and medication administration record.
• Communicate changes in resident condition to the Registered Nurse, physician and family as appropriate.
• Follow infection control, safety and regulatory requirements governing long-term care practice.

Education & Experience
