Job summary

Talented-Medical-Solutions is seeking an Licensed Practical Nurse (LPN) for a Long Term Care setting in Wisconsin. The role delivers direct nursing care to residents under RN or physician supervision, administers medications, monitors residents, and delegates tasks to Certified Nursing Assistants as appropriate.

The ideal candidate is a graduate of an accredited practical or vocational nursing program with licensure as an LPN/LVN in the state and BLS certification.

Qualifications

• Graduate of an accredited practical or vocational nursing program is required.
• Prior experience in a long-term care or skilled nursing setting is preferred.

Responsibilities

• Administer medications and treatments in accordance with physician orders and facility policy.
• Perform resident assessments and monitor for changes in physical, cognitive or behavioral status.
• Supervise and delegate appropriately to Certified Nursing Assistants on the assigned unit.
• Complete accurate and timely documentation in the resident record and medication administration record.
• Communicate changes in resident condition to the Registered Nurse, physician and family as appropriate.
• Follow infection control, safety and regulatory requirements governing long-term care practice.
