Qualifications

• Education from an accredited school of nursing resulting in LPN licensure.
• Current LPN license in the practicing state.
• CPR/AED certification required or obtainable within six months.
• IV certification required or obtainable within six months.

Responsibilities

• Provide basic nursing care and assess residents on admission and ongoing.
• Communicate significant changes to IDT and document assessments.
• Participate in care planning and update plans as interventions revise.
• Implement nursing interventions, administer meds and treatments safely.
• Administer IV fluids only if IV certified and document promptly.
• Maintain composure during crises and use SBARR for physician contact.
• Support QAPI process and address problem areas promptly.
• Assist residents with ADLs and respond to call lights.
