Job summary

Complete Care at Fox Hill is seeking a dedicated LPN Licensed Practical Nurse for direct care on 2nd shift (full-time) and 3rd shift (part-time). PRN with experience may be considered. You will administer medications, monitor residents, and support care planning in a skilled nursing setting.

Qualifications

• Licensed to work within this state as a LPN Licensed Practical Nurse.
• Able to pass background check, drug screen, professional references check & physical evaluation.
• Professional & clinically skilled with leadership qualities.

Responsibilities

• Administer medications (oral, topical, IV, injectable).
• Perform wound care/prevention and manage pain.
• Monitor health status and track changes with data analysis.
• Provide catheter/ostomy care and collect specimens as needed.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers and others.
• Educate residents and families about health status and care expectations.
• Maintain detailed documentation of health status and care progress.
• Oversee and delegate CNA tasks to ensure quality care and teamwork.

Here's What You'll Do for Our Residents:

Administer Medications: Give oral, topical, IV, injectable medications

Wound/Skin Care: Perform wound care/prevention, manage pain, apply dressings

Monitoring: Observe physical and mental health status- track changes & analyze data

Clinical Procedure: Provide catheter/ostomy care, gather specimens etc.

Care Planning: Assist in executing individualized care

Collaboration: Strategize care with physicians, therapists, social workers & others

Family & Resident Education: Explain health status, changes and expectations for care

Documentation: Keep a detailed record of health status, all care, progress & interactions

Team Oversight: Oversee & delegate CNA's ensuring quality care/teamwork
